The Consolidated Plan is a concise summary of the actions, activities, and the specific federal and non-federal resources that will be used each year over a five-year period to address the priority needs identified by the Consolidated Plan Priorities Survey.
The Consolidated Plan is carried out through the Annual Action Plan which is the written plan, including any amendments, that describes how the county will use this funding to help primarily low and moderate-income residents within a specified program year.

Accomplishments and progress toward the Consolidated Plan goals are reported in the Consolidated Annual Performance and Evaluation Report (CAPER) on an annual basis.
