To complete any of these forms on your computer, fill in the information, then save the file to your computer. You can then print and fax it to 908-575-3965 or email to surrogatesoffice@somersetcountynj.gov.
If you have any questions, please email the Surrogate's Office or call 908-231-7003. Office hours are 8:30 a.m. and 4:30 p.m. weekdays excepting holidays, and until 6 p.m. on Wednesdays. Office visits are by appointment only.
The Surrogate’s Office is located in the Somerset County Administration Building at 20 Grove Street, Somerville, NJ 08876.
